The Clinical Care Facilitator is an advanced Clinical Nurse, proficient in the delivery of complex nursing care, who leads the practice of nursing and manages care in accordance with principles of the care delivery model, on the nursing unit. The clinical care facilitator (CCF) functions within the roles of clinical expert and lateral integrator, clinical leader, and outcomes manager. The CCF is recognized as a professional role model and clinical expert, and promotes a professional environment that supports nursing excellence and a professional practice model. The CCF reports to the unit nursing director. Bachelor's degree in Nursing is required. Approximately three to four years of acute care experience in the clinical specialty. Must demonstrate clinical maturity and expertise. Excellent interpersonal skills, including collaboration and negotiation skills. Effective oral and written communication skills. Team building skills. Licensure as a registered nurse in the District of Columbia is required. BLS required. Certification in clinical specialty area preferred.
